I inherited this coin from my late grandfather. It appears to be a Spanish colonial real but I haven't found another coin that has the reverse. It's a bit larger than an American quarter and sounds like it's silver.
Is this just an old necklace piece, a token/medallion, or is it a real coin? Any assistance is greatly appreciated.

Many would call these a "Love Token"…

Looking closer at the obverse, this is what they called a "Pin Back", which was worn on the lapel.
